Online Application
Please complete your grant application via our online application (required). All applications received by the established deadline via the Foundation's online application process will be reviewed by the Foundation. Please click on the link below to be directed to the online application:


Required Attachments
As part of the online application, applicants are asked to provide the following required documents:
  • IRS Determination Letter certifying the organization’s current 501(c)(3) tax-exempt status.
  • Organization's annual operating budget for current fiscal year.
  • Project or program budget (if not a general operating support request).
  • Listing of board of directors and affiliations.
  • QMAP results (youth mentoring applicants only, see below)
These documents may be submitted as attachments, via the online application. The Foundation no longer accepts mailed attachments.

The Foundation may request a site visit and/or additional information relevant to your proposal, such as recent financial statements. All applicants will be notified funding decisions following a board meeting.

Youth Mentoring Grant Applications
If your organization is applying for a youth mentoring grant, the Foundation requires completion of the Mentoring Partnership of Minnesota's Quality Mentoring Assessment Path (QMAP) before an application may be submitted. A summary of your organization's QMAP results should be attached to the application. Application Deadlines
The Foundation board meets three times each year in April, July and October. Grant applications must be received by January 1, April 1, or July 1, to meet the deadlines for the respective meetings.

July 1 late October / early November

Because of the high volume of funding requests received, the Foundation often must decline to support proposals of obvious merit, which are either outside the Foundation’s guidelines or beyond its available resources.
